Career Opportunity Development, Inc. (CODI) provides intensive case management services to individuals with a history of mental illness. CODI provides case management services and assists participants with accessing public entitlements, obtaining employment, and living independently.

 

CODI provides Community Support Services that include Case Management and Life Skills Training. 

This Application is for Support Services ONLY and does NOT include housing.

Community Support Services are available to individuals who 

1. Are 18 years or older,

2. Are living in stable housing in Atlantic County, NJ.

3. Have a documented severe and persistent Psychiatric Diagnosis.

CODI can ONLY accept applications for individuals with a psychiatric diagnosis.

The psychiatric diagnosis must be supported with documents verifying the diagnosis.

By signing the application, the applicant acknowledges that there is no guarantee regarding the availability of support services.  CODI does not discriminate in the application process in accordance with all state and federal guidelines. 

 

Applicants are responsible for notifying CODI of any changes in information.

You will receive an automatic reply via email confirming that we have received your application for housing or services. If you do not receive a confirmation email, please re-submit your application via the CODI website.  We will contact you within 10 business days of receiving your application.

90-day period.
