Community Support Services for Individuals with Mental Health Diagnoses
Career Opportunity Development, Inc. (CODI) offers intensive case management services to individuals with a history of mental illness. Our team assists participants in accessing public benefits, securing employment, and developing the skills needed to live independently.
CODI provides Community Support Services (CSS), which include personalized Case Management and Life Skills Training designed to enhance daily functioning and promote overall well-being.
Please note: This application is for Support Services Only and does not include housing assistance.
Eligibility Requirements for Community Support Services:
Applicants must meet the following criteria:

Be 18 years of age or older

Reside in stable housing within Atlantic County, NJ

Have a documented severe and persistent psychiatric diagnosis

Submit the following documentation:

Birth Certificate
-
Valid, unexpired government-issued photo ID
-
Social Security Card
-
Copy of health insurance cards
-
Important: CODI can only accept applications from individuals with a verified severe and persistent psychiatric diagnosis. Supporting documentation is required.

Submitting an application does not guarantee the availability of support services.

Please Note: This is application is for Support Services Only and does not include housing. If you would like to apply for housing, please apply for Shared Semi-Independent Housing or Opioid Use Disorder housing.

CODI complies fully with all state and federal non-discrimination laws throughout the application process.

Applicants are responsible for notifying CODI of any changes to their personal or contact information.

You will receive an automatic email confirmation upon submission of your application. If you do not receive a confirmation, please re-submit your application through the CODI website.

CODI will respond to your application within 10 business days of receipt.

If you do not hear from us within 10 business days, please contact our main office at 609-965-6871.

Applications remain active for 90 days. A new application must be submitted after this period.
