Community Support Services (CSS)
CODI provides case management services to individuals with a documented severe and persistent mental health diagnosis. Our team assists participants with accessing benefits and developing the skills needed to live independently in a community setting.
Community Support Services include:

Personalized Case Management

Life Skills Training to support daily functioning and overall well-being
Please note: This application is for Support Services only. It does not include a housing voucher.
Eligibility Requirements
Applicants must:

Be 18 years of age or older

Reside in stable housing within Atlantic County, NJ
-
Have a documented severe and persistent mental health diagnosis
Required Documentation

Birth Certificate
-
Valid, unexpired government-issued photo ID

Social Security Card
-
Copy of health insurance card(s)
Please note: CODI only accepts applications from individuals with a documented severe and persistent mental health diagnosis.

Applying does not guarantee services.

This application is not for housing. To apply for housing, please use the applications for Shared Semi-Independent Housing or Opioid Use Disorder Housing.

CODI complies with all state and federal non-discrimination laws.

Applicants must notify CODI of any changes to personal or contact information.

Upon submission, applicants will receive an automatic confirmation email. If a response is not received, please resubmit it through the CODI website.

CODI will respond to applications within 10 business days. If no response is received, please get in touch with our main office at 609-965-6871.

Applications remain active for 90 days. After that, a new application must be submitted.
