OPIOID USE DISORDER (OUD) SUPPORTIVE HOUSING APPLICATION
Career Opportunity Development, Inc. (CODI) provides housing and intensive case management services to individuals with a severe history of opioid use who are homeless or at risk of homelessness in New Jersey.
CODI assists individuals in locating and securing housing and also provides case management services. The staff works with participants to access public entitlements and obtain and maintain employment. CODI’s Peer Recovery Mentor partners with individuals on their way to recovery from opioid use to pursue their self-expressed goals and maintain stable housing.
OUD Supportive Housing is available to individuals who
1. Are 18 years of age or older,
2. Have a documented serious history of an opioid use disorder, and
3. Have documentation of homelessness or the risk of homelessness.
CODI can ONLY accept applications for individuals with an Opioid Use Disorder (OUD).
By signing application, the Applicant acknowledges that there is no guarantee regarding the availability of housing or support services. CODI does not discriminate in the application process in accordance with all state and federal guidelines.
Applicants are responsible for notifying CODI of any information changes.
You will receive an automatic reply via email confirming that we have received your application for housing or services. If you do not receive a confirmation email, please re-submit your application via the CODI website. We will contact you within 10 business days of receiving your application.
If you have not been contacted within 10 business days, please call CODI main office at
609-965-6871. Applications will be active for 90 days and must be re-submitted after the
90-day period.
